Before You Apply

Before submitting your LTSS application, it helps to confirm you likely meet eligibility requirements. Review our RI LTSS Eligibility Guide or call us for a free pre-application benefits check.

Pro Tip from Our Team

Many families wait too long to apply because they assume they won't qualify. The sooner you apply, the sooner care can begin. If you're unsure, call us first and we will tell you honestly whether it's worth applying.

The Application Process: Step by Step

1

Confirm Eligibility

Review the income limits, asset rules, and functional need criteria. You must be a RI resident, meet Medicaid financial thresholds, and need help with daily activities. Call us for a free check — 401-402-0076.

2

Gather Your Documents

Having your documents ready speeds up the process significantly. You'll typically need:

  • Government-issued photo ID (driver's license or passport)
  • Social Security card and Medicare or Medicaid card
  • Proof of income (Social Security award letter, pension statements, pay stubs)
  • Bank account statements from the last 3 months
  • Proof of RI residency (utility bill or lease)
  • Medical records or doctor's letter documenting care needs
  • List of current medications and diagnoses
3

Submit Your Application

You can apply in several ways:

  • Online: Apply through HealthSourceRI at healthyrhode.ri.gov
  • Phone: Call RI DHS at 1-855-MY-RIDHS (1-855-697-4347)
  • In person: Visit your local DHS office
  • Mail: Download and mail the paper application

Keep a copy of everything you submit and note the date. Ask for a confirmation number when applying by phone.

4

Complete the Home Assessment

After your application is received, a care coordinator from the state will contact you to schedule an in-home assessment. This visit evaluates your functional needs and determines how many hours of care per week you qualify for.

Be honest and thorough during this assessment. Describe your worst days, not your best days. If you need help on bad days, say so.

5

Receive Your Authorization

If approved, you'll receive a Notice of Decision outlining your authorized care hours and start date. You'll then have the right to choose your home care agency.

6

Choose In Home WellCare as Your Provider

You have the right to select your preferred home care agency. Many RI families choose In Home WellCare because of our CHAP accreditation, behavioral health CNA team, and personalized care approach. Simply let your care coordinator know your choice.

How Long Does It Take?

RI Medicaid is generally required to process applications within 45 days for standard cases, or 90 days if a disability determination is needed. Home care typically begins shortly after approval and provider selection.

In urgent situations where someone is at risk, the process can sometimes be expedited. Call us and we can help advocate for faster processing if needed.
