Long-Term Services & Supports (LTSS) is Rhode Island's Medicaid-funded program that helps elderly adults and people with disabilities get the care they need to live safely at home or in the community — instead of moving to a nursing facility.
LTSS is administered by the Rhode Island Executive Office of Health and Human Services (EOHHS) and covers a wide range of services, including professional home care from agencies like In Home WellCare.
The goal is simple: help you or your loved one stay at home, stay independent, and maintain quality of life with the support of trained caregivers.
What Does LTSS Cover?
Personal Care Assistance
Bathing, dressing, grooming, toileting, and mobility support
Homemaker Services
Light housekeeping, laundry, meal preparation, and errands
Companion Care
Supervision, socialization, and emotional support
CNA / Skilled Care
Certified Nursing Assistants for medical-level personal care
Respite Care
Temporary relief for family caregivers
Day Programs and Transportation
Adult day health programs and medical transportation
Who Is LTSS For?
LTSS is designed for Rhode Island residents who need ongoing support due to age, disability, or chronic illness. Generally, you may qualify if:
- You are 65 or older, or have a qualifying disability
- You need help with daily activities like bathing, dressing, or meal preparation
- You meet Rhode Island Medicaid income and asset requirements
- You want to remain at home rather than move to a nursing facility
- You are enrolled in or eligible for RI Medicaid (RIte Care / Medicaid)

How LTSS Works with Home Care
Once you are approved for LTSS, the state authorizes a certain number of care hours per week based on your assessed needs. An approved home care agency like In Home WellCare is then assigned to provide those services.
Apply & Get Assessed
Submit your LTSS application. A care coordinator visits to assess your needs and determine approved hours.
Choose Your Agency
You have the right to choose your home care provider. Many families choose In Home WellCare for our CHAP accreditation and personalized care.
Care Begins
Your caregiver starts on your schedule. LTSS pays the agency directly with no out-of-pocket cost to you.
What Does LTSS Cost?
For eligible Medicaid recipients, LTSS home care services are covered at no cost to you. Medicaid pays the home care agency directly for the authorized hours of care.
